Pulmonary valve obstruction during cardiac catheterization.

چکیده
OBSTRUCTION of a stenotic pulmonary valve by the exploring cardiac catheter, a serious and not infrequent complication of cardiac catheterization, has received little attention in the literature.1-4 Experiences in this laboratory indicate that pulmonary valve obstruction can occur both in a manifest, symptomatic form and in a concealed, asymptomatic form in patients with severe pulmonary stenosis. As catheterization studies are extended more generally to the infant age group and performed more often without anesthesia, the high incidence reported in this study may be more generally observed. The purpose of this communication is to present clinical and physiologic observations during pulmonary valve obstruction and to derive from these a concept of the precarious hemodynamic status that exists in severe congenital pulmonary valvular stenosis.

Different Presentation of Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery in Adults: Case Reports
Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital cardiac malformation. We report three cases of ALCAPA who survived to adulthood. The first case was a 51-year-old woman who complained of typical chest pain that was diagnosed with ALCAPA using cardiac catheterization and coronary computed tomographic angiography (CTA). The second case was a 30-...
متن کاملUltrasound in the diagnosis and evaluation of therapy of idiopathic hypertrophic subaortic stenosis.
Twenty patients with idiopathic hypertrophic subaortic stenosis (IHSS) were studied with cardiac echography. Eight of these patients were studied during left heart catheterization. The echographic pattern of mitral valve motion during systole was altered in a characteristic manner when hemodynamically significant left ventricular outflow obstruction was present in the eight patients studied at ...
متن کاملProblems in the diagnosis and surgical treatment of pulmonic stenosis with intact ventricular septum.
Obstruction to pulmonary blood flow may occur in the pulmonic valve, in the infundibulum or in both. Cardiac catheterization aids in the determination of the site of obstruction. Criteria for the differentiation at operation of valvular and infundibular pulmonic stenosis are enumerated, and the usefulness of accurate pressure tracings during operation is emphasized. The accurate identification ...
متن کاملIntegrated Percutaneous Atrial Septal Defect Occlusion and Pulmonary Balloon Valvuloplasty
Introduction: Atrial Septal Defect (ASD) is one of the most common congenital heart diseases during childhood. Today, ASD closure is done by occlutech device via cardiac catheterization. ASD repair with transcatheter technique has shown high closure rate. However, coexistence of severe pulmonary stenosis and large defect in atrial septum is rare. When these two problems coexist, the result is i...
متن کاملUses and fallacies of intracardiac electrograms at the pulmonary valve.
Intracardiac electrograms by cardiac catheterization were first recorded by Lenegre and Maurice (1945). The electrographic patterns in various chambers of the heart were studied by Hecht (1946), Battro and Bidoggia (1947), and Kossmann et al. (1950), and were mainly of value in studying the activation of the heart. Their importance during cardiac catheterization was first shown by Emslie-Smith ...
